Site Map

Register for a card at any Mid-York member library. You can use it to borrow materials held by the libraries. The 13 digit card number which appears after the A on the card can be used to log in to the Mid-York Web Catalog and Electronic Resources, one day after you receive your new card.

Application Forms for Library Cards

Links to the forms are listed below next to the library name, for those libraries that provide them. Please print, fill out and bring the form to the library along with identification, preferably identification with a photograph. The age cutoff for the youth application varies by library and may require the presence of a parent or guardian.

The library name is linked to contact information: library address, telephone number.
The forms are in the Adobe PDF format. If you do not have the PDF reader, you may download it free from www.adobe.com/
