CHE-GA-QUAT-KA

The Dunham Public Library presently has many of the Whitesboro High School's yearbook, Che-Ga-Quat-Ka, but we are missing the following years:

1937, 1943, 1967-1971, 1975-1981, 1984, 2001, 2002, 2004 - Present

If you have a yearbook for any of these years and would be willing to donate it to the library, please call 736-9734

We wish to thank the Whitesboro Alumni Association for their recent donation of yearbooks.

The Dunham Public wishes to thank Mr. John J. Salerno for his donation of several yearbooks on 8/11/06

Access income tax forms online:

Each year, the Dunham Public Library provides a full selection of reproducible Federal and State tax forms. Since the forms arrive at irregular intervals, we may not have a particular tax form available when library patrons are looking for them. However, the forms and instructions are available online. Please ask at the Information Desk, if you would like assistance in accessing them.

Library patrons now have the option of accessing and printing off State and Federal tax forms on their own. Click on the websites in the table below:

health care information...

Important health information in light of recent events:

This booklet from the Attorney General's Health Care Bureau is a timely reminder that "Preparing a few simple legal forms known as 'Advance Directives' can help ensure that your wishes are respected and your health care stays in the hands of people you trust."

In New York State, advance directives include: Health Care Proxy, Living Will and Do Not Resuscitate Order (DNR). The booklet describes advance directives in detail and discusses how New York State law applies.

"Planning Your Health Care in Advance" (OCLC #58592849) is a New York State paper document depository item. Online editions are available from the Department of Law Health Care Bureau at:

http://www.oag.state.ny.us/health/health_care.html

and in the New York State Library's digital document management system at:
